Health Navigator predicts and prevents avoidable urgent and emergency care. This means addressing the £6bn problem of vulnerable patients that go back to A&E again and again while not receiving the care they really need. We do this by combining the power of predictive AI with nurse-led patient health coaching and behaviour change. Through locally trained predictive algorithms operating daily on Trust servers, we identify patients with a high probability of non-elective bed-day consumption.
Our registered nurses on-site then recruit these vulnerable patients to a 4-6 month proactive health coach intervention that measurably improves patient outcomes and delivers significant urgent and emergency care activity and cost savings. Results can be impressive with up to 59% decrease in unplanned admissions, 52% reduction in A&E attendances and close to 40% reduction in total hospital costs for patients that receive our intervention.
This means an ROI for CCGs that can often exceed 250%. We are proud of our determination to evidence the impact of our work and are delivering a 3,000 patient multicenter randomised control trial in collaboration with the Nuffield Trust and NIHR. Health Navigator cares for some 3,000 patients across 7 NHS Acute sites and 9 collaborating CCGs.
