HomeLink Healthcare is a clinician-led, specialist Hospital at Home and Virtual Ward service provider. We have been delivering safe, high-quality services to NHS patients in the place they call home since 2016.
By working in partnership with hospitals, consultants, community providers and GPs to provide ‘out of hospital’ care we prevent some people being admitted to hospital and enable others to come home more quickly.
HomeLink Healthcare provide Hospital at Home services through a number of pathways including Virtual Wards, Early Supported Discharge, Discharge to Assess, Re-ablement, Rehabilitation, Anticipatory Care and Bridging Packages of Care. Our services offer great benefit to patients, reduce pressures and help save the NHS money.
We set up our first Virtual Ward in 2019 at the Norfolk & Norwich University Hospital. HomeLink Healthcare follows and adheres to the Virtual Ward guidance provided by NHS England.
- An on-site team, managed by a Clinical and Operational Lead, co-ordinates patient care from the hospital, liaising with the Consultant.
- A community-based team of Nurses, Physiotherapists and Healthcare Support Workers deliver in-person care in the place people call home. Typically, visits are scheduled between 0700 and 2200hrs, seven days a week.
- Continuous or intermittent remote monitoring communicates patient information to the hospital. We have relationships with a number of leading providers of remote monitoring equipment, both intermittent and continuous, which can be included into our service. Alternately, we are happy to use equipment already purchased by the NHS.
- Real-time reporting enables performance to be regularly tracked against key performance indicators (KPIs).
- The Virtual Ward is supported by a 24/7 clinical on-call service.
