Our highly skilled multi-disciplinary nursing and therapeutic teams can deliver safely a range of interventions in the patient’s home, seven days a week. We use remote patient monitoring technologies. These provide effective monitoring and data-driven learning for the well-being of patients and effectiveness of the health system.

We do this by working flexibly through the following clinical pathways:
- Virtual wards: Patients who are medically stable can come out of hospital sooner to continue their pathway to full recovery in the place they call home, rather than in hospital. This may include nursing and/or physiotherapy care.
- Early supported discharge: Patients who are medically stable can come out of hospital sooner to continue their pathway to full recovery in the place they call home, rather than in hospital. This may include nursing and/or physiotherapy care.
- Reablement: Using a person-centred approach, individuals learn or re-learn the skills necessary to be able to engage in activities/occupations that are important to them.
- Admission avoidance: Active treatment by healthcare professionals in the patient’s home for a condition that otherwise would require acute hospital in-patient care, always for a limited time period.
