As trusted system partners, our purpose is to work collaboratively with each of our community members to improve the health and wellbeing of their population. In particular, we provide access to a methodology and infrastructure that enables the creation, dissemination and maintenance of condition-specific pathways of care.
HealthPathways includes starter pathways, supporting infrastructure, and online platforms for front line clinicians to use at the point of care. This collaboration delivers better patient outcomes by reducing variation in pathways and clinical practice and enables new clinical evidence and health policy to flow swiftly into practice.
