This talk will explore the NHS England-supported gastroenterology GPwER (GP with extended role) training programme which is developing clinicians who can help to deliver the NHS’s desired left-shift of care from hospital to community within gastroenterology.
This programme follows the national GPwER in gastroenterology training framework co-authored and approved by the BSG and PCSG in October 2024, and a full evaluation of the programme including an independent cost analysis has been carried out to evaluate the potential benefits of this role for the wider healthcare system which will be discussed further during this talk.
Current GPwER-led services nationally provide a model for the role, and these services will be discussed further during this talk.

Upper aerodigestive symptoms in general and dysphagia in particular, are nonspecific and poorly-localising. Significant proportions of patients are therefore investigated by several disciplines and there is a notable incidence of itnerspecialty missed cancers.
We have created an integrated pathway with input from gastroenterology, upper gastrointestinal surgery, ENT surgery, and respiratory medicine to help standardise diagnostic pathway initiation. This treats upper aerodigestive presentations as “nonspecific” symptoms and uses the approach that has been successfully adopted by the Nonspecific Symptoms Pathway.
Using mindfulness-based patient support, leveraging frugal digital innovations, and harnessing advances in endoscopy technique, Transnasal Panendoscopy provides a one-stop assessment of the full path of swallowing in awake patients attending outpatient appointments, avoiding a general anaesthetic and a sedation procedure.
