Claire Hopkins

Claire joined RIVIAM Digital Care in 2016 and leads the discovery, design, development and implementation of RIVIAM’s care co-ordination services for NHS Trusts, Local Authorities and private healthcare organisations where she enables transformation and successful outcomes for staff, clinicians, GPs, partners and patients.

She works closely with BaNES Council and 26 health, social care and third sector  delivery partners at the BaNES Community Wellbeing Hub who use RIVIAM’s multi-agency referral management and care co-ordination services to enable personalised, preventative care.

Over her career, Claire has led the design and delivery of business-critical digital programmes and technologies. She has done so for both private and public sector organisations including for Dell, Hewlett Packard, BT, UK Government Department for Business Innovation & Skills and Department for Education. Her work has enabled business growth, transformation, the achievement of efficiencies and successful outcomes.

Claire has MBA with Distinction from University of Bath School of Management.
