Clare Evans

As Programme Manager for Care Co-ordination & Hospital@Home I’m responsible for developing these services to support patients receiving the most appropriate care closer to or at home. We work alongside South West Ambulance Service to assess patients where an out of hospital pathway maybe a more suitable alternative to conveyance to Emergency Departments or admission.

As a Nurse Consultant I’ve developed new services and ways of working across the acute sector and community services and more recently worked as Deputy Director for Transformation at Health Innovation West of England.

I’m passionate about enabling and delivering real change to optimise the quality and efficiency of the services we can offer to our population.
