Currently Head of Integrated Urgent Care Team, a team consisting of nurses, therapists and social workers across the community and acute. I am a registered nurse that has worked across acute and community teams. Starting off on a respiratory ward then heading into the community as a community staff nurse.

I began to understand the impact of Long term conditions and the prevalence of high-intensity users across primary and secondary care. I then was seconded to set up an Admission Avoidance team, this was successful and then to lead the implementation of the Home first pathway across the acute hospital into the community team. The IUCT team have been successful at being finalists for 4 HSJ awards and were winners for 2. The teams are continually looking for innovation to support patients including a Covid Rehab Pathway and NWAS handover on scene, these initiatives are all part of the wider NHS Long Term Plan.
