NHS Population Health Management Conference

Population Health is an approach that aims to improve physical and mental health outcomes, promote wellbeing, and reduce health inequalities across an entire population.

A relatively new approach called Population Health Management (PHM) is helping the NHS understand our current, and predict, our future health and care needs so actions can be made to tailor better care and support with individuals, design more joined-up and sustainable health and care services and make better use of public resources.

PHM is a partnership approach across the NHS and other public services including councils, the public, schools, fire service, voluntary sector, housing associations, social services, and police. All have a role to play in addressing the interdependent issues that affect people’s health and wellbeing.

For example, adults and children who live in cold, damp housing may be more likely to develop respiratory problems over the next 20 years because their lungs are affected by the mould spores in their homes. If we improved their housing now, by working with partners such as, local councils and housing associations, they may not end up with various health conditions in the future which can result in poor quality of life (conditions like asthma, chest infections, and other respiratory problems) and could avoid the need for multiple health and care services.

Implementing population health management services is a key focus within the NHS long term plan and is supporting ICSs’ to understand the areas of greatest health need and match NHS services to meet them. These solutions are becoming increasingly sophisticated in identifying groups of people who are at risk of adverse health outcomes and predicting which individuals are most likely to benefit from different health and care interventions, as well as highlighting health inequalities.

Our upcoming conference aims to provide updates and clarity on this developing strategy and share best practices and outcomes from across the NHS Ecosystem.

Long term conditions like Diabetes Mellitus are a cause for significant expenses for the healthcare system. Prevention of the onset of such conditions and even delaying the progress to complications can present a cost-saving opportunity and quality improvement in the lives of patients. Increasingly, research has shown the efficacy of AI in managing long term conditions. In this presentation, I will discuss developments in this area and showcase applications in this field. ** 5 Minute Q&A included **

Developing a system-wide culture to Enable Population Health Management

Ayesha Janjua

Effective Population Health Management (PHM) is pivotal to the way health and care organisations work together to improve the health and wellbeing of their populations, a core strategic aim for all ICSs. In this session, we will discuss how ICSs can create a healthy culture that drives effective PHM. We will share our experience of supporting systems (and their component organisations) to address issues around culture, relationships and ways of working to drive population health approaches and develop effective skillsets that are crucial for an enabling and positive culture. We will also explore concepts of ‘stewardship’ and value-based population health that are critical to the system-wide adoption of PHM. ** 5 Minute Q&A included **
