The idea of virtual ward as it operates in Medway NHS Foundation Trust (MFT) is a concept that breed the use of technology to offer hospital treatment and care to patient, at the comfort of their own homes.

It is a hospital level care that make us of electronic monitoring device to offer remote monitoring, while offering best of care. SMART virtual ward is unique in MFT because it is multidisciplinary, involving all the medical and surgical teams working together to offer acute response, recovery and rehabilitation to patient who are ordinarily should have been admitted in hospital bed; hence the name SMART (surgical medical acute recovery/rehabilitation/recovery team).

This has successfully kept patients out of hospital, reduce front door pressure in A and E and acute areas, decongest in-patient beds, free up hospital bed for more acutely unwell patient and ensure hospital avoidance- especially with our community virtual ward team in Medway and Swale. This model has also enabled the team to established various patient centred pathways, that allow acutely unwell patient to receive hospital level treatment and care without a necessary need for hospital admission, while using safety net as a key for escalation in event of deterioration.
