In this session, Josefa will take you through the journey of Integrated Neighbourhood Team (INT) development, reflecting on progress made, key learnings, and the road ahead. The session will explore the origins of INTs, highlighting early partnership-building across health, local authority, and the Voluntary, Community, Faith, and Social Enterprise (VCFSE) sector. It will also examine how INTs have evolved to align with the six core elements of the Neighbourhood Health Guidance 2025/26, including population health management, modern general practice, wider community partnerships, social care integration, proactive care, and workforce leadership.
Despite progress, sustaining INTs presents challenges such as funding gaps, data-sharing barriers, sector alignment, and workforce capacity constraints. The session will showcase practical strategies, including joint funding bids, enhanced digital integration (MOSAIC & LCR), and strengthened cross-sector collaboration, to overcome these barriers. Attendees will gain real-world insights and actionable solutions to support the continued transformation of neighbourhood-based care.

Integrated Neighbourhood Teams (INT’s) are one of the many shifts in health and social care to support improving outcomes, prevention, shifting resources into the community whilst improving health inequalities.
For INTs to work all stakeholders will need to let go of old models of working and build strong relations and trust across their systems.
Those we are serving need to be at the heart of every decision and be involved in the decision making, it’s their life and their community.
We will need a multidisciplinary approach from all services in all sectors and move towards a teams of team approach.
The concept of a "team of teams" in Integrated Neighbourhood Teams (INTs) refers to a collaborative approach where multiple smaller teams work together as a cohesive unit. This structure aims to:
Enhance Collaboration: By bringing together various professionals from different disciplines, INTs foster better communication and teamwork.
Streamline Processes: Simplifying referral and administrative systems reduces barriers to care and improves efficiency.
Improve Patient Outcomes: Coordinated care ensures that patients receive comprehensive and timely support, addressing both medical and social needs.
Leverage Expertise: Teams can draw on specialist knowledge from various fields, providing more holistic and specialised care.
This approach helps create a more integrated and effective healthcare system, benefiting both patients and providers, whilst putting patients first.
