Current Landscape

The Integrating Care Conference will explore how systems aim to seamlessly connect healthcare providers to deliver coordinated, patient-centred care. Emphasizing collaboration and interoperability, ICS initiatives foster partnerships between hospitals, primary care, community health services, and social care agencies. As part of the Labour government's "Build an NHS Fit for the Future" manifesto, they will trial Neighbourhood Health Centres, integrating services like family doctors, district nurses, care workers, physiotherapists, palliative care, and mental health specialists under one roof to improve patient treatment.  As digital technologies advance, ICSs leverage data analytics, telehealth, and interoperable electronic health records to enhance care coordination, resource allocation, and population health management to improve serivces and patient outcomes.

Importance and timeliness of the event

The integrating care conference will delve into the pressing need for cohesive healthcare systems. In today's landscape, where healthcare is multifaceted and interconnected, the demand for seamless integration among various providers and organisations has never been more crucial. This conference offers a platform to address these challenges head-on, focusing on how integrated care systems (ICS) strive to bridge the gaps between different healthcare entities to provide more personalised and efficient care to patients. With a spotlight on collaboration and interoperability, the conference highlights the evolving landscape of healthcare partnerships, encompassing hospitals, primary care providers, community health services, and social care agencies. In an era propelled by digital innovation, the conference spotlights the pivotal role of data analytics, telehealth solutions, and interoperable electronic health records in revolutionising care co-ordination, optimising resource allocation and enhancing population health management. This conference stands as a beacon for the transformative potential of integrated care in shaping the future of healthcare delivery.

Case Study - Royal Voluntary Service

Are you an ICS that is looking to improve patient experience, efficiency and workforce capacity? The NHSCVR programme - a unique partnership between NHSE, a charity (Royal Voluntary Service), and a tech platform (GoodSAM) - have over 45,000 volunteers supporting over 100,000 patients, and this grows month-on-month. The programme is a free resource for healthcare systems and patients to tap into.  Volunteers can expedite discharge from hospital, support patients with practical tasks, provide welfare support via telephone calls or drop off/return equipment for/from virtual wards. These are existing 'integrated neighbourhood teams' of volunteers already supporting your local healthcare system - come find out how we can further embed and scale this programme within your ICS to meet your priorities.

Alison Tonge
Executive Director of Strategy and Innovation
Arden Gem

The NHS is a complex system – as we aspire to deliver better integrated care, we need to have a dynamic planning approach that enables transparency between the care we plan and deliver and the capacity, workforce, supplies, facilities needed.  We need to be able to understand interdependencies and drivers and most importantly how we measure our resources and performance across a team, division, organisation or population and system.  In this presentation we will explore the challenges and opportunities in this goal of dynamic connected planning and some case studies and learning.
